A072 SEND Transition Review Lead job summary

We are recruiting a SEND Transition Review Lead to support students with SEND as they move into college and prepare for their next steps. The role leads transition planning and EHCP reviews, working closely with students, families, schools and partners to create personalised, supportive pathways for success.

About You

  • Experience supporting young people with Special Educational Needs and Disabilities (SEND).

  • Confidence working with a range of professionals, agencies and external partners.

  • A good understanding of the SEND Code of Practice and the Children and Families Act (2014).

  • An HND, degree, or equivalent qualification, or relevant professional experience.

  • Strong IT skills, including Microsoft Office (Excel and Access).
