SEND TA job summary

At Chaddesden Park Primary school we are looking to recruit a passionate, dedicated and caring SEND TA to support a Year 6 pupil on a one to one basis. The job involves supporting the child with their Educational Health Care Plan, following the planning of the teacher, setting up personalised provision and feeding back to staff and parents, also keeping up to date records of the child's performance. This role would also involve working with external agencies and following up with programmes and interventions specific to the child. This role would be the supporting the child to access provision during at least the morning sessions.
We're looking for someone who is patient and caring and can foster good relationships with both children, families and staff.
